Create the Custom Survey Type

After creating a custom facility survey, create the survey type, including facility information and records.

Complete the following steps to create a custom survey type:

1. Navigate to Type on the Navigation pane and click in the banner.

2. In the New Facility Survey window, you have the option to create a new Facility Type or use a saved template.

   1. If creating a new facility survey, refer to Create a New Custom Facility Survey.

   2. If creating a facility survey from a template, refer to Create a Facility Survey from a Template.
