Create a New Custom Facility Survey

Complete the following steps to create a new custom facility survey:

1. In the Type folder, click in the banner to create a new custom facility survey.

2. To create a new facility survey not based on a saved template, select *New*from the drop-down list.

3. Select a facility type from the drop-down list or enter a name for the Facility Type in the text field.

4. If you selected *New *as the Facility Type, enter a name for the Facility Subtype in the text field.

5. If you selected a template as the Facility Type, select an option for the Facility Subtype.

6. Continue with Add Facility Information to add facility information to your survey.
