Create a Facility Survey from a Template

Complete the following steps to create a new custom facility survey using a saved template:

1. In the Type folder, click in the banner to create a new custom facility survey.

2. To create a new facility survey based on a saved template, select the template name from the drop-down list.

3. Select a facility type from the drop-down list or enter a name for the Facility Type in the text field.

4. Continue with Add Facility Information to add facility information to your survey.
